Vaccine Information Statements
Vaccine Information Statements (VISs) are statements describing vaccines that are covered by the National Childhood Vaccine Injury Act (NCVIA). By Federal law, VISs must be produced by the Centers for Disease Control and Prevention (CDC) and cannot be altered.

Vaccine Administration Record for Children and Teens (Exit Site)
Before administering any vaccines, give the parent/guardian all appropriate copies of VISs and make sure they understand the risks and benefits of the vaccine(s). Update the patient’s personal record card or provide a new one whenever you administer vaccine.
